Investigation Summary
On February 9, 2011, Employee # 1 was working as a carpenter installing sheeting on the roof. Employee # 1 slipped on fell approximately 25 to 30 feet from the roof of a single family house to the concrete ground at the back of the house. Employee # 1 sustained broken mandible, lung contusion, broken and damaged teeth and lip injury. Employee # 1 was transported to Scripps La Jolla hospital by air.
