Employee Loses Toe in Accident

At approximately 1;30 p.m. on July 22, 2010, Employee #1 was sliding an iron gate valve across a pallet when he lost his balance and took his hands off of the gate valve, causing it to fall over. The gate valve landed on Employee #1's second toe, on his right foot, initially causing bruising. Approximately a month and two weeks after the initial injury, the toe required amputation due to lack of circulation.
