Investigation Summary

At approximately 2:00 p.m. on March 2, 2010, Employee #1 was hanging papers on a wall. Employee #1 used a small child's chair made of plastic with a metal frame as a step to get up onto a table approximately three feet high. To get down from the table, Employee #1 attempted to step back onto the child's chair. As Employee #1 stepped down, she missed the chair, falling approximately three feet to the ground and landing on her left leg. Employee #1 was transported to Kaiser Zion Hospital in San Diego, CA. diagnosed with a broken left leg in three places, and was hospitalized for over 24 hours. Employee #1 worked at Vista La Mesa Elementary School located in La Mesa, CA. The employer notified the Division of the serious accident on March 3, at 9:05 a.m. The reporting delay did not fulfill the eight-hour reporting requirement; however, the employer was not aware the employee had been admitted to the hospital until after 6:00 p.m. on March 2.
