Event: 11/20/2009
Employee Amputates Finger When Caught in Machine

At approximately 8:00 a.m. on November 20, 2009, Employee #1, a temporary machine operator, was working on an Enformer EF-11, Model Number C-85, and Serial Number 2651, nipple sizing machine to widen the ends of catalytic converters. A catalytic converter part became stuck in the machine, and Employee #1 attempted to remove it without the use of an extension tool. He struck the switch, and it engaged the nipple widener. His right index finger got caught between the widener part of the machine and the nipple end of the converter part. Employee #1 was transported to a nearby medical center, where the surgeon was unable to reattach the amputated right index finger.
