Investigation Summary
At approximately 5:30 p.m. on June 30, 2009, a maintenance worker employed at Poway Unified School District was stripping floors during summer recess at Westview High School. The worker operated a Tennant 3500, Wet/Dry Vacuum (Model Number 607673, and Serial Number 10081680). While performing his task he slipped and fell and struck his head on the wet floor. He suffered a head injury and became unconscious. He was taken to Pomerado Hospital in Poway and was later transferred to Palomar Hospital in Escondido, CA, where he was hospitalized with a concussion.
