Employee Is Injured When Hand Caught by Corregator Rolls

At 3:45 p.m. on February 14, 2012, Employee #1 came to work early that day and was assigned the task of training Employee #2 on how to operate the Single Face Machine. Employee #2 and other machine operators had cleaned the machine from 9:00 p.m. until 1:30 a.m. Employee #1 arrived to work at 1:00 a.m., and assisted with the cleaning until approximately 1:30 a.m. Heat was restored to the 615 Corrugator machine, and Employee #3 instructed the employees to start the machine. As the machine was starting to run, Employee #1 notice ink was spewing from the Print Coat Section of the machine. Employee #1 took a soapy cloth rag and began to wash the print coat die as it ran production through the machine. The rag and Employee #1's right hand became caught between the two rolls. While Employee #1's hand was still stuck, he reached over with his left hand to turn the machine to "off" at the control box. Employee #1 suffered a right hand avulsion to his 1st, 2nd, 3rd and 4th fingers. Employee #1 was hospitalized.
