Employee Suffers Injury Falling from Ladder

At approximately 1:20 p.m. on November 30, 2011, Employee #1 fell about 5 ft from a ladder at a construction site, and fractured his left foot. Employee #1 was transported to a local hospital for medical treatment. At time of accident, Employee #1 was standing on a 10 ft ladder inspecting and verifying the torque of hilti-bolts, on ductwork under the ceiling, and fell when ladder became unbalanced. Employee #1 stated that ladder had safety pads under feet, had no missing rings, no slippery conditions on floor, and was stable enough when he climbed on it. Employee #1 was over reaching the tipping point of ladder as he applied pressure to the torch wrench. Due to Employee #1's off balance position and body weight, the ladder collapsed causing him to loose balance and fall.
