Investigation Summary
At approximately 12:30 p.m. on November 16, 2011, Employee #1 was feeding corrugated sheets of cardboard into an EMBA 240 machine. The corrugated sheets were approximately 15 in. by 35 in. The EMBA machinery comprised of four sections, which included a feeder, printer, slotting and gluer and was used in the construction of corrugated cardboard boxes from the sheets. On the last run of the day, the last sheet became stuck on the feeder. Employee #1 manually pushed the sheet down into the feeder and through an unguarded point of operation on the machine, when his right hand was caught by moving parts and pinned between the rollers of the machine. Employee #1 sustained crushing hand injuries that included, carpometacarpal joint dislocation, abductor pollicis brevis muscle tear, thumb intrinsic opponens pollicis muscle tear, common digital artery damage, dorsal radial ligament tear, right common digital nerve damage and dorsal soft tissue avulsion defect. A supervisor, who was in the shipping area at the time, was notified of the accident. He directed other employees to summon emergency medical personnel. Two maintenance mechanics disassembled the feeder section of the EMBA machine to remove Employee #1's hand. Employee #1 was transported to a medical center, where he underwent surgical procedures for the hand crushing injuries and was hospitalized for postoperative care.
