Employee Amputates Finger in Moving Machine Parts

At approximately 9:30 a.m. on August 28, 2011, Employee #1, a temporary employee of 8 months, was repairing a 120-volt carton taping machine. The power supply was on, and Employee #1 saw a loose wire inside of the machine. He put his right hand inside to reach the wire, but his finger got caught in the moving chain and gears of the machine, resulting in an amputation of his finger. Employee #1 stated that he was not trained in lock out/tag out safety rules from the on-site employer. Investigations revealed that a hazardous energy control procedure was not utilized when Employee #1 was engaged in repair work on the carton taping machine. The employer was cited for these violations.
