Employee's Finger Is Lacerated, Later Amputated

At approximately 11:15 a.m. on June 29, 2011, Employee #1, a material handler, was using a wood shaper in the mill department. He was not properly trained on the use of the wood shaper machine when cutting short plexi glass parts. He was operating a Powermatic wood shaper, Model Number 028, Serial Number 0028019. He was using the Shaper to cut 0.25-in. plexi glass parts. He disengaged the feeder guard of the machine, a common practice for Employee #1 when he cut short plexi-glass parts. While the wood shaper was running he began to sweep excess debris with his right hand into the vacuum system, located directly behind the rotating blade of the bull nose. He did not stop, deenergize or disengage the power source, nor did he lock or block out to prevent inadvertent movement of a machine. His glove was caught by the rotating blade which pulled his finger into the point of operation. He received a severe laceration to the right ring finger with bone loss of the distal phalanx. Employee #1 was hospitalized and his right ring finger was amputated.
