Investigation Summary
On May 20, 2011, Employee #1 was holding an apron when it was caught in the chain-sprocket drive of a running cardboard tape packaging machine on Salad Line Number 3. She attempted to pull the apron away, but her left finger was caught in the pinch point of chain-sprocket drive, resulting in a fingertip amputation. The pinch point on the machine was not covered or guarded and was located at an approximate height of 4 ft. Employee #1 was transported to a medical center, where she received treatment on the injured fingertip and was then released.
