At approximately 1:15 p.m., on March 17, 2011, Employee #1 of Seal Methods Inc., suffered a serious crushing injury to 3 fingers on her right hand when the top die of a Preco punch press came down and crushed her fingers during some adjusting work on a plastic liner in the danger zone of the machine.
