Employee Trips and Fractures Hip

On February 12, 2011, an employee, a machine operator, was turning her position to walk to another location. She fell when her right foot caught between the legs of a two-step step ladder she had placed there for access to a machine's hopper. The employee was admitted to the hospital, received surgery on her fractured hip and remained in the hospital for greater than 24 hours. The employee had placed the step ladder at the Deburring Stamper Machine in order to view the hopper. She also used the ladder to sit on when she manually worked the Slotter Machine and occasionally used it to access the top of the two machines for cleaning.
