Investigation Summary

Investigation Nr: 202476743
Event: 05/21/2010
Employee suffers partial amputation of index finger

At 9:30 am, on May 21, 2010, Employee #1, a plumber of Baker Commodities Inc., was working alone in the parking lot of a restaurant. His assigned task was to collect grease from a grease trap of a restaurant, cleaning the drains with a high-pressure water jet system and doing any required maintenance. On this day, he had difficulty lifting the lid of a grease trap. Employee #1 made various attempts and during the course of his work, he needed to move the trailer of the high pressure water jet which was hitched behind his vehicle. Investigation thus far indicates that a stabilization bar/ jack which held up the trailer hitch failed, and the hitch fell down on his right index finger, causing a fingertip amputation. Employee #1 was transported to Pomona Valley Hospital for treatment.
