SIGN FELL FRM CEILING, HIT CUSTOMER & CASHIER ON HEAD

On June 14, 2010 at approximately 2:45 pm E1 was injured while working for HK2 of Rowland Heights LLC (HK2) at work location 18414 E. Colima Rd. Suite #8, Rowland Heights, CA 91748. The Division was notified by the employer and the Los Angeles County Fire Department in a timely manner. The incident occurred inside a grocery store involving a display sign that fell on a patron and E1 who was working as a cash register clerk. E1 was transported to UC Irvine Medical Center where she was treated for minor injuries and released in less than 24 hours. For unknown reasons, the wire rope slipped through the friction-type locking mechanism of the hardware and resulted in the suspended sign to fall to the ground. All 24 wire rope to hardware attachments both on the roof joists and sign's body did not have wire rope end stops to prevent the wire rope from slipping through the friction locking mechanism. There were no indications the wire rope itself failed. There were no indications the wood screw attachments failed. The only evidence present is the wire rope slipping through the locking mechanism.
