Investigation Summary
Around 11:00 a.m. on January 20, 2010, Employee #1 and a coworker were moving an office desk from the sub-roof on to the elevated pallet of a forklift through the 59-inch-wide unguarded section of roof perimeter. Employee #1 fell 107 inches to the floor below near forklift and suffered a serious head injury. Employee #1 was not tied off at that time with a safety belt, a lanyard or any other approved fall protection system to prevent him from falling down. Employee #1 was hospitalized for few nights at USC Medical Center in Los Angeles. Employer reported the accident to Cal/OSHA within eight hours-time frame, required by regulation. An investigator from Cal/OSHA West Covina Office initiated an inspection of the place on January 21, 2010 with a management official at site. Primary employer, DMS Facility Services, leased a couple of temp. employees to work at the facility of, Sargent Fletcher Inc. dba Cobham Mission Systems, El Monte, California, for cleaning and janitorial services. The employer was cited for safety violations relating to issues on fall protection.
