Investigation Summary

Investigation Nr: 202475752
Event: 04/29/2009
Employee Amputates Thumb Tip

At approximately 10:30 a.m. on April 29, 2009, Employee #1, a laborer, was performing his duties as laborer at Sonoco Products Company in City of Industry, CA. He attempted to clear the inlet of a running Vaughan centrifugal vertical wet well chopper pump (Model Number V4K-063, Serial Number 64344) located in the pulper conveyor pit. Employee #1 had been assigned to clean the pulper conveyor pit and had removed most of the paper debris with a shovel. Employee #1 was washing the pulper conveyor pit when the chopper pump inlet became jammed with debris. As he cleared the pump with his gloved right hand, he contacted the unguarded spinning impeller blade, partial amputating his right thumb. Employee #1 was transported to a medical center, where he received treatment and was hospitalized. Upon investigation, the employer did not ensure that the Vaughan centrifugal wet well chopper pump was guarded to prevent the employee from reaching into the point of operation. In addition, the employer did not ensure that employees were trained on the use and maintenance of such tools, methods or means to protect employees from injury.
