Employee Amputates Finger While Operating Machine

At approximately 12:20 p.m. on April 5, 2009, Employee #1 was removing a rag resting on the top of a Husky injection molding machine (Model Number X- series, Serial Number 11183). Employee #1 used the ladder built into the back of the Husky injection molding machine to access the working platform located 50 in. above the shop floor. This working platform is located 33 in. below the top of the Husky injection molding machine. Employees typically used this working platform to access the plastic hopper located on the top of the machine. Employee #1 sustained the amputation of her right pinky finger, when her finger entered the zone of danger created by the shear point between a moving machine cam and the machine frame. Employee #1 was hospitalized.
