Employee's Finger Is Caught in Machine and Is Amputated

At 4:15 p.m. on March 16, 2009, Employee #1 was cleaning a hydraulic pump, Graco Bulldog/King Pump model number 208-356, in the wash area of a bakery with a water hose. He noticed a jammed food piece in between the valve and cylinder assembly of the pump. The pump was operating hydraulically by airhose pressure of about 10 psi, at the time of the accident. He tried to flick off the jammed food piece from the valve and cylinder with his index finger, but his finger got caught between the valve and cylinder. His right index finger was partially amputated. Employee #1 was taken to Foothill Presbyterian Hospital, where he underwent about four hours of surgery and was hospitalized. The subsequent investigation revealed that the employee did not use a hand extension tool for flicking off the jammed food piece from machine, as required.
