Investigation Summary
At approximately 12:41 p.m. on March 3, 2009, Employee #1, a carpenter, was moving a roof truss on a two story commercial building. The roof truss was placed the day before in the wrong position. The employee fell approximately 25 ft, through a 40 in. wide by 40 ft long hole. The hole had been cut in the North side of the second floor below his working area where the truss was located. Scaffolding had been removed from this area one and half hours prior to the accident. Employee #1 sustained serious injuries including a fractured pelvis and was hospitalized. It was determined that the incident was caused by the employer's failure to provide covers over open holes in the worksite. As a result, a serious citation Title 8 CCR 1632 was issued to the employer.
