Investigation Summary
On August 26, 2008, Employee #1 was showing a trainee how to use the Cincinnati Press Brake, Model Number CB175. Employee #1 had the laser guard off so he could bend small parts faster because the machine dies if you are taking your time. Employee #1's fingers were caught in the press and amputated.
