Employee Is Killed When Caught in Machine

On January 14, 2009, Employee #1 and a coworker, a control operator, were performing maintenance work on a jump chain, while changing out an air valve. When Employee #1 signaled to the coworker to start the machine, his jacket was caught in a spinning shaft, pulling him into the machine. Employee #1 was killed. The safety procedures were not utilized.
