Investigation Summary
At approximately 5:15 p.m. on September 30, 2010, Employee #1 was operating a table saw to make a cut. The saw blade measured 24 inches in diameter. As Employee #1 activated the foot pedal and used his right hand to guide the material towards the blade, his hand contacted the blade and amputated three of his fingers. Employee #1 was taken to a nearby medical facility, where he was hospitalized and treated for his injury.
