Investigation Summary

Investigation Nr: 202520326
Event: 05/22/2010
Employee suffers fractures from a tractor tire

On May 22, 2010, Employee #1, a tractor operator of Double Diamond Dairy was closing the gap between two rear tractor wheels so that the wheels would align up with the corn rows. On this day, Employee #1 was performing this task on a newer tractor that had never had its wheels spacing moved in or out. As he was using an air wrench to loosing the last lug bolt, the wheel became free and fell onto Employee #1. Employee #1 sustained a compound fracture and two additional fractures to their right leg. The task Employee #1 was performing was a common task and had been performed numerous times in the past. Unbeknownst to the employer and employees, this new model of tractor did not have a safety pin built into it like the older models, which prevented the wheel from coming off the axle.
