Employee Injured in Forklift Accident

At approximately 4:00 p.m. on August 19, 2010, Emplyee #1 was driving a forklift when his prescription eyewear fell. Employee #1 looked down for a second to adjust his glasses, and struck a building support pole, resulting in laceration injury to the forehead.
