At approximately 4:40 pm, on June 2, 2010, Employee #1, a temporary service employee of Kingspan Insulated Panels, Inc., was cleaning peel coat from rollers of a conveyor belt when the machine started. The shirt of Employee #1 was pulled into the rollers, pulling in his left arm, resulting in crushing injury. Employee #1 was hospitalized due to his injuries.
