At approximately 10:47 p.m. on October 28, 2009, Employee #1 bypassed a photoeye on a "C Cab Headliner Speaker Assembly Fixture." A pneumatic arm was activated, resulting in an avulsion (i.e., amputation) of her left hand middle finger.
