Employee Is Injured in Fall from Ladder

On February 17, 2009, an employee was descending down a 28 foot extension ladder when the locks disengaged allowing the ladder to collapse. The locks passed up 2 to 3 rungs before locking again. As the ladders locked back up, it jerked causing the employee to be thrown from the ladder. The employee fell approximately 14 to the ground. The employee was hospitalized and treated for multiple unspecified fractures.
