On October 22, 2008, Employee #1 placed her hands in the vicinity of the discharge port of a Blend-Tech Brand dual-shaft Paddle mixer. The tip of Employee #1's left index finger was amputated, when her finger was caught in the mixer's unguarded point of operation. Employee #1 was hospitalized and treated for his injuries.
