Investigation Summary
On November 5th 2008, Employee #1 was bending a sheet of metal on a Verson Press Brake when his right index finger got caught in the point of operation. Amputation at his first knuckle occurred. Employee #1 was transported by the employer to the hospital for treatment of his injury. Re-attachment of his finger was not able to be performed.
