Employee's Eye Is Injured by Flying Object

On October 23, 2008, Employee #1, a service technician, was hoisting a 35-lb canister with a rope and brass-coated snap-hook. When the snap-hook failed in its neck area, the rope and a portion of the snap-hook recoiled and struck his right eye. Employee #1 was hospitalized for treatment, including surgical removal of the damaged eye.
