Investigation Summary
On August 20, 2008, Employee #1, a crew leader, was showing an employee how to operate the OK Superformer box machine. Employee #1 saw that the box just exiting the box maker had not been properly taped. The team leader reached in behind the guard and grabbed the next box to be taped and, as they were attempting to pull the box out, the machine cycled causing the next box to push the one they were grabbing forward. Employee #1's right hand was under the box as the taper activated. Employee #1's right ring finger tip was amputated by the blade of the tape cutter. Employee #1 was hospitalized.
