Employee's Finger Is Amputated by Car Wash Machine

On April 24, 2008, Employee #1 was performing maintenance on a car-washing machine. As Employee #1 was standing on top of a ladder, at approximately 4 feet, he slipped or fell off the ladder. During the fall, Employee #1 caught his left pinky finger on the machine, and his finger was amputated at the lower knuckle. Employee #1 was hospitalized.
