Investigation Summary

Investigation Nr: 200651545
Event: 03/19/2009
Employee Is Killed When Struck by Flying Object

At approximately 11:30 a.m. on March 19, 2009, Employee #1, a gang saw operator, was operating a Cornell Model Number 642 combination gang saw and edger. He was feeding cant logs into the gang saw side of the equipment. The 4 ft cant was set up to saw into 3 boards. A coworker worked the off-feed side as the board extruded from the saw. The equipment was equipped with various pressure rollers, saw cover and kickback mechanisms. The board to the outside edge of the blades hung up. On the off-feed side, two boards extruded by 2.5 ft, with the third board only extruded approximately 1 ft. Some type of jamming occurred approximately once per day. The coworker was holding the three boards while Employee #1 tried to unjam them. Employee #1 moved to the other side of the feed chute, adjacent to the operating controls, when the cant ripped apart. Pieces of one board were ejecting backwards, striking Employee #1 in the sternum, killing him.
