Investigation Summary
On August 24, 2008, an employee and a coworker were replacing two multi-piece rim wheels on a trailer. They had removed two wheels and were putting a new wheel back on the trailer. The employee, tightening the thimbles by hand, was killed when the tire exploded and threw him across the floor. The coworker was walking away at the time to get some supplies and was not injured. The tires had a capacity of 120 psi, but were reportedly only inflated to approximately 30 psi, at the time of the accident.
