Investigation Summary
On August 12, 2008, Employee #1 was spotting for a crane operator excavating inside a coffer dam. He was standing at the southeast corner of the coffer dam. He approached the swing area of the crane and was crushed between the cranes counter weight and the steel sheeting of the coffer dam. Employee #1 was killed.
