Investigation Summary
On September 9, 2008, Employee #1, a 43-year-old male, with Curtis Contracting Inc, was working with a coworker who was backing a Ford tandem wheel dump truck to an excavator that was going to load broken concrete onto it. The truck struck Employee #1 who was walking in the same direction that the truck was backing, running over the right portion of his body which resulted in his death.
