Investigation Summary
On June 30, 2008, an employee was in the area where 5-ft by 13-ft steel form, weighing 1,200 lbs was being delivered by Maitowoc Model 4100W, 250 Ton Crane. The form became disengaged from crane hook while being placed on the deck. The form struck the employee and knocked him into other equipment. The employee was hospitalized and treated for fractures in chest.
