Investigation Summary

Investigation Nr: 202468989
Event: 07/18/2010
Employee's Thumb Is Amputated in Unguarded Machine

On July 18, 2010, Employee #1 was operating a Techno-Maiz tortilla-making machine. The machine was a CNC fully automatic machine, manufactured in Mexico and did not conform to Cal-OSHA guarding requirements. The employer had manufactured Plexiglass guards for exposed openings of sprockets and chain drives. The guard had three keyholes cut into it. Employee #1 noticed dough getting stuck on the rollers and gears. The guard was not in place, and she placed her hand in the opening and suffered a partial amputation of her right thumb.
