Investigation Summary
At approximately 8:30 p.m. on June 12, 2010, Employee #1 sustained serious occupational injuries, when he fell from a ladder onto concrete floor. At the time of accident Employee #1 was ascending a fixed ladder used for climbing up on mezzanine. Employee #1 was transported to the Hospital and underwent surgery.
