Employee Sustains Electric Shock While Replacing Bulb

At 1:30 p.m. on January 27, 2010, Employee #1 was on Skyvan bucket truck (Model Number TS33HO, Serial Number B3488-2-37, manufactured August 1988, working height of 28 ft) replacing a light bulb on an exterior flood light. When he made contact with a 277 volt phase-to-ground wire, he received an electrical shock to his left middle and ring fingers. This caused his left upper arm to forcibly contract and dislocate his shoulder. Employee #1 was treated at San Joaquin Hospital for the dislocation, and then admitted to Grossman Burn Center for treatment of the finger burns. Employee #1 failed to deenergize the light bulb socket prior to work.
