Employee Sustains Crushed Hand by Falling Aluminuim Molding

"On March 1, 2010, Employee #1 and a supervisor were using a five-ton bridge crane (Konescranes General Hoist, Serial Number 13981) with a chain to lift a 2,073-pound aluminum mold into an injection molding machine (JSW Model Number J440SAII, Serial Number 3 285740511). The mold was going to be used to make 10-gallon plastic frames for aquarium tanks. The chain was attached to the mold by two 0.625-inch hoist rings screwed into the holes on the plates of the mold. The mold was lifted up and into the injection molding machine by the supervisor by means of a radio controls for the crane. Employee #1 was standing on the side of the injection molding machine turning the mold to fit it into the machine. One of the hoist rings pulled out of the mold causing the mold to fall, crushing the right hand of Employee #1. Employee #1 was not a contract employee. The failed hoist ring was rated to 4,000 pounds and was required to be torqued to 60 feet-pounds. Employee #1 was hospitalized for nine days."
