Investigation Summary

Investigation Nr: 202468856
Event: 01/26/2010
Employee Is Injured While Lifting Mirror

At approximately 1:00 p.m. on January 26, 2010, Employee #1 was while unloading a mirror from a beveling machine in Valencia CA 91355. Employee #1 was moving 3 feet by 6.5 feet mirrors as they came out of the Bovone Beveller type Mini Maxi 371 S that beveled the 6.5 feet long bottom edge. Employee #1 would unload the mirror off of the track as they came out of the beveling machine and move it to a washing station. Employee #1 was unloading by gripping it with his two gloved hands from the top of the mirror. The track the mirror was on is 2.5 feet off of the ground so the employee had to reach slightly overhead to lift the mirror off of track. Employee #1 was wearing thick rubber gloves (Make: Best Model: Nitty Gritty 66NFW-10), and protective sleeves (Supplier: C. R. Laurence Model Number: GWS8354). The gloves have a safety cuff that extends slightly over the sleeves. When Employee #1 reached up to grab the mirror, the sleeve lowers below the wrist. The safety cuff on the glove fits loose on the wrist. The mirror went between glove and sleeve causing the laceration on the wrist. The Los Angeles County Fire Department reported the accident to the Van Nuys District office at 3:32 p.m. on January 25, 2010. The Employee #1 was taken to Henry Mayo hospital by ambulance.
