Employee Is Injured in Fall from Ladder

On November 16, 2009, Employee #1, with the William S. Hart Union High School District, was placing several hand-wipe boxes in a three-level pallet storage rack that was approximately 12 ft high. After placing some boxes in the first rack level, he set up an 8 ft ladder (Werner Type A fiberglass 2001/2002, 300lbs. capacity) against the second level's bottom support. Employee #1 leaned the ladder against the rack because there was insufficient room in the aisle to open it into its A-frame configuration. While on the ladder pushing a box into the second rack level, he fell backwards to the floor. Employee #1 was hospitalized at Henry Mayo Hospital for fractures of his knee and femur.
