Investigation Summary
On May 14, 2009, Employee #1, with Western Precooling Systems, was working on the "Transfresh" conveyor system, where the pallets of fruit were packaged and sealed. Employee #1 was standing on a platform on the east side of the roller and was using a heavy duty stapler to staple the plastic sheet to the bottom of the pallets on one side. A coworker was performing the same type of stapling on the other side. Employee #1 was having trouble with his stapler, and he decided to cross over the conveyor to get another stapler. Employee #1 stepped onto the turntable portion of the conveyor to ride to the other side, and his left foot became caught between the turning table and the stationary metal cover on the conveyor at the transition from the rollers. Employee's left big toe was amputated to the last knuckle and the second toe lost the nail. He was hospitalized for about 12 hours and had surgery on his toes.
