Employee Falls from Ladder and Fractures Leg

On March 2, 2009, Employee #1 was installing metal straps at a second floor level, approximately 8 ft high. The employee was using a 15 ft extension ladder for access and for installing the straps. He placed the ladder on top of a stack of about 30 pieces of .5 in. plywood, and leaned the top portion against the wood framing at the second level. He climbed the ladder to see if it was okay. When he took a step to descend the ladder, the ladder slid down and he fell 7 ft to the ground with the ladder. Employee #1's left leg was fractured, and he was hospitalized and for two days. A holding frame was placed in his leg, and surgery was performed two weeks later.
