Employee's Hip Is Dislocated in Platform Fall

On March 18, 2009, Employee #1 and a coworker were working on an elevated platform. They were repairing a roll-up door using a forklift and elevated platform. The platform was not properly attached and fell. Employee #1 was hospitalized with a dislocated hip.
