Employee's Finger Tip Is Amputated in Resistance Welder

On June 2, 2008, Employee #1 was placing a stock in a resistance welder. For an unknown reason, the machine cycled in an opposite direction and amputated the tip of Employee #1's right middle finger. The detect-a-finger guard was adjusted to allow for the width of the stock. This prevented the guard from sensing the finger in the point of operation.
