Investigation Summary
Employee #1 was working on an elevated platform with another employee. The platform was located in Bay Number 14 of the plant. The employees were working to cut apart an existing bag house located on the platform. The employees were in the midst of a thirty-minute cool-down period prior to lunch. During the cool-down period, the employees were throwing smaller pieces of cut metal down to the ground into an adjacent shot blast chamber where a metal hopper was placed. One employee heard a sound behind him and, upon turning, saw that Employee #1 was no longer on the elevated platform. The employee went to the edge near the point where he had last seen Employee #1 and looked down. He saw Employee #1 lying on the grating that comprises the floor of the shot blast chamber. Employee #1 sustained an unspecified fracture and was declared dead at the site.
