Investigation Summary

Investigation Nr: 202505384
Event: 06/30/2008
Employee Sustains Lacerations in Fall from Roof

Employees were removing the existing air handler unit by cutting it into smaller pieces on the rooftop using an electric saw. Employees would then take the pieces to the edge of the rooftop and throw them approximately 13 ft 6 in. to the ground. The employees had a horizontal rope installed by tying one end to a concrete column, then running the rope approximately 60 ft across the rooftop and wrapping it around two vent pipes. The employees were wearing safety harnesses while working near the edge of the rooftop throwing the old air handler pieces to the ground. Employee #1 was not secured to the life line and when throwing a piece of equipment down to the ground he lost his footing and fell approximately 13 ft 6 in., landing on a piece of debris. Employee #1 sustained unspecified lacerations and was hospitalized.
